SAMPLE FORMS AVAILABLE FROM TCSG

TCSG maintains a "Forms Clearinghouse" to make sample forms available to those working on Title III-funded law and aging projects. TCSG has collected forms from around the country and selected for distribution those that illustrate best practices for delivering legal services. Two to three selected forms are available in each of the following categories:

(1) Client Contributions;

(2) Client Intake;

(3) Measuring Client Satisfaction;

(4) Contracts between Providers and Area Agencies;
(5) Community Education and Publicity;
(6) Evaluation and Monitoring;
(7) Internal Grievance Procedures;
(8) Office Procedures and Operation;

(9) Reporting;

(10) Requests for Proposals; and

(11) Retainer Agreements. Most forms can be supplied at no charge. For some of the longer forms, it is necessary to charge a small amount to help cover costs. For more information and an order form, call TCSG at (734) 665-1126.
